The smart car certainly is distinctive, and if you do all city driving the small size can't be beat. However, I have heard on the highway it isn't so great, it gets blown around a bit by the wind coming off other cars. Plus, I don't think it has been around long enough to get a good idea of its reliability.

If you do Highway driving, you could consider the Toyota Yaris, starts around 12k gets 29/36 mpg or the Honda Fit at $15,300 gets 28/35mpg.

Then of course their are the hybrids. You have to do a lot of driving to make up for the extra cost, but the Prius gets 48/45mpg and the Civic Hybrid gets 40/45mpg

I think most people who want a small fuel efficient car will do well with the smart car if they do all city driving or the Yaris if the do any amount of highway driving.
